The Department of Health and Human Services recently revealed a new program that tests the efficiency and affordability of considering the health-related social needs of Medicare and Medicaid beneficiaries.
The five-year program, called the Accountable Health Communities Model, will offer funding of as much as $157 million to as many as 44 bridge organizations through cooperative agreements. This program is the first of its kind offered by the Centers for Medicare and Medicaid Services Innovation Center.
The organizations will explore, through a screening assessment, three different options for addressing the health-related social needs of beneficiaries. The three options are community referral, community service navigation and community service alignment.
