Study finds nicotine withdrawal increases pain sensitivity and need for opioids after surgery

Researchers from Fudan University Minhang Hospital and Shanghai Eastern Hepatobiliary Surgery Hospital have identified a link between nicotine withdrawal in abstinent smokers and increased pain sensitivity. The study, published in JNeurosci, was led by Zhijie Lu and Kai Wei.

The research compared 30 abstinent smokers with 30 nonsmokers. Findings showed that those who had stopped smoking experienced changes in specific brain areas, heightened pain sensitivity, and required more postoperative pain relief—especially opioids—than nonsmokers. The increase in pain sensitivity was tied to the duration of abstinence; the longer individuals refrained from smoking, the more sensitive they became to pain. This effect appeared within a particular period after quitting and previous studies suggest that pain sensitivity may return to normal if abstinence lasts longer than three months.

Distinct sets of brain regions were associated with both the increased need for postoperative care during withdrawal and the symptoms experienced during nicotine abstinence. According to the researchers, "The relationship between postoperative care requirements and withdrawal symptoms from abstinence was linked to a different set of brain regions."

Further investigations are underway into alternative postoperative pain relievers that could be more effective than opioids for those who have quit smoking. The team is also studying how preoperative nicotine replacement therapies might help manage these effects.
